Pharmacy ReturnsI am pleased that you have selected Quality Rx Returns to process your expired drugs for credit. In order to help you get started, we have compiled the following guidelines designed to help you minimize any delays or problems in getting the full and immediate benefits from our program.

Step One:
Complete the enclosed Program Terms & Registration Form. It is extremely important that you complete this form accurately. All of this information is necessary to ensure that your Returns are processed promptly and efficiently to maximize the amount of credits on your expired drugs.

If you are Doing Business As (DBA) other than as shown on your DEA Registration, record both names on the Quality Rx Returns Registration form. Whether you mail or fax these forms you must include a copy of your current DEA Registration and State Pharmacy license along with the completed Program Terms & Registration Form which will be held for future reference. Once this information is received, we will assign you a five digit account number. You must use this account number whenever you communicate with us or ship expired drugs to us for processing.

Should your account information change (i.e. address, wholesaler, etc…), contact us immediately to update your records to ensure you continue to receive the proper credits. We are not responsible for credit lost due to failure to notify Quality Rx Returns of any change in account information.

Step Two:
Class II Drugs.  These must be handled separately and require DEA Form 222 instructions to be complied with as well. Enclosed is a separate set of instructions about how these are to be handled. Failure to conform to these guidelines will delay our ability to process these returns.

Class III-V Drugs These must be handled in accordance with the enclosed guidelines and a ‘Scheduled Pharmaceuticals Packing Slip” must be included with the shipment. It is critical that this form be completed accurately. This is required by state and federal regulatory agencies. Failure to comply with these instructions will only delay the processing. Included in this packet you will find instructions about the correct procedure for packing, shipping, etc. these substances.

Non-Scheduled Drugs: Please complete the pharmacy record information (NDC, Quantity and Expiration Date) on the enclosed “Non-Scheduled Pharmaceuticals Packing Slip”. The information provided on a packing slip will allow Quality Rx Returns to file an insurance claim for the value of the shipment in the unlikely event a package be lost during transit.

Step Three:
Follow all the instructions published in the enclosed “Processing Guidelines” with respect to how the products are to be shipped according to the Classification (Scheduled — Non-Scheduled —. Class II) they fall in. Observing these directions will save time and avoid errors.

Step Four:
Once your items are packaged up, affix a red FedEx Ground shipping label on each container. Write your account number in the “RA#” section and schedule the pick-up by calling: FedEx(888) 777-6040 or QRX (727)499-2211.
