Class II Drugs Processing Instructions

The following procedure must be complied with at all times for processing Class II drugs.

  1. Pharmacist must complete and forward “Class II Analysis Form” to Quality RX Returns by fax (865)-951-2497 or mail (address below). Complete this form accurately and completely with all required information (i.e. NDC, quantity, etc.). Only this form is to be sent to Quality Rx Returns. No drugs are to be shipped until advised.
  2. We will review the data on the form and will send you a DEA 222 to ship the expired Class II drugs for processing.
  3. Review the DEA Form 222 for accuracy. If you find any errors, please contact Quality Rx Returns. Complete the information that must be provided only by the pharmacist shipping the Class II drugs. This section is identified and titled as “to be filled in by the supplier”. The information that you must provide is:
    • DEA Registration Number for the Pharmacy.
    • The National Drug Code Number (NDC).
    • The exact number of container(s) shipped which should match the number of packages on the left. If not shipping an item, enter “0” in the column.
    • The date of the shipment.
  4. Retain the “Brown” copy of DEA Form 222 at the pharmacy for your records.
  5. Mail the “Green” copy of DEA Form 222 to your local DEA office not later than the 15th of the month following the date that the drugs were shipped to Quality RX Returns for processing.
  6. DEA Form 222 is valid for a maximum of 60 days from the date Quality RX Returns issued the form to the pharmacy. This date is located at the top of the form. If the drugs are not shipped to us within this 60-day period, the form is invalid and must be voided and returned to Quality RX Returns.
  7. Ship only drugs listed on the DEA Form 222. You can not send more or less of the items listed on the form.
  8. Make a copy of DEA Form(s) 222 and place this in each container(s) as a packing slip. A pre-paid FedEx Ground shipping label(s) will be sent with DEA Form 222’s.
  9. Place all Class II drugs to be shipped into the enclosed tamper-evident bag and complete the information on the attached label before placing them into the corrugated box. Class II drugs must always be packaged separately from non-scheduled and Class Ill-V drugs. DO NOT USE THE BAG AS A STAND-ALONE SHIPPING CONTAINER
  10. Write your Account ID Number on each box/carton. Number boxes consecutively (1 of 3, 2 of 3, 3 of 3, etc.). This will ensure Quality Rx Returns can account for every container shipped.
